Which County Handles Smyrna Birth Records
Smyrna is in Rutherford County, and the Rutherford County Health Department is the local office for vital records. Phone: (615) 898-7880. Hours: Monday through Friday, 8:30 AM to 4:00 PM. The department can issue certified birth certificates for any Tennessee birth through VRISM, the statewide electronic system linking all 95 county health departments.
One important note: the Rutherford County Health Department has records for births on or after 1949 in its local system. For births before 1949, you need to contact the Tennessee Office of Vital Records in Nashville, which holds records going back to 1908.
Smyrna does not have its own birth records office. All requests go to the county or state level. The town is served by the same county resources as Murfreesboro, La Vergne, and other Rutherford County municipalities. How to Request a Smyrna Birth Certificate
Three options are available for Smyrna residents.
In person: Go to the Rutherford County Health Department during business hours. Bring a government-issued photo ID. Complete Form PH-1654, the standard Tennessee birth certificate application. Pay $15.00 per certified copy. Most walk-in requests are handled the same day.
By mail: Download the Form PH-1654 application. Fill it out completely, attach a copy of your photo ID, and include a check or money order for $15.00 payable to the Tennessee Department of Health. Send everything to the Tennessee Office of Vital Records, 710 James Robertson Parkway, Nashville, TN 37243. Processing typically takes one to three weeks.

Tennessee Birth Certificates and Court Records
Birth records in Tennessee are vital records held at the state level. They are not court records and are not accessible through the court system. If you need a certified copy for a legal proceeding, you must obtain it through the Tennessee Office of Vital Records or the county health department, not through a courthouse.
Courts may require a certified birth certificate as part of certain legal proceedings including probate, adoption, name change petitions, and passport applications. The certified copy must bear the official state seal to be accepted. Photocopies do not count as certified records for legal purposes.
Who Can Request a Smyrna Birth Record
Tennessee restricts access to birth records less than 100 years old. This applies to all Rutherford County births, including those in Smyrna.
Eligible requesters include the person named on the certificate (if 18 or older), a parent listed on the record, a legal guardian with court documentation, a spouse, a child, or a sibling of the named person. Legal representatives with written authorization may also apply. You must provide a valid government-issued photo ID. If requesting on behalf of someone else, bring proof of your relationship such as a birth certificate, marriage certificate, or court order.

Statewide birth registration in Tennessee began in 1908. For Rutherford County births before that year, the main resource is the Tennessee State Library and Archives. The Tennessee State Library and Archives (TSLA) at 1001 Rep. John Lewis Way N., Nashville holds early vital records, church registers, and family papers for the county. TSLA is open Tuesday through Saturday, 8:00 AM to 4:00 PM.
The Tennessee Early Vital Records Application (TEVA) is a free online database covering birth records from 1908 through the early 1910s.
