How to Request an Oak Ridge Birth Certificate

Three options are available: in person, by mail, or online.

In person: Visit the Anderson County Health Department at 710 N. Main Street in Clinton. Bring a valid government-issued photo ID. Fill out Form PH-1654 at the office. Pay $15.00 per certified copy. Most in-person requests are handled the same day. Oak Ridge residents should note that Clinton is the county seat and where the health department is located, about a 15-minute drive from Oak Ridge.

By mail: Download the Form PH-1654 application. Complete it, attach a copy of your photo ID, and include a check or money order for $15.00 payable to the Tennessee Department of Health. Mail to: Tennessee Office of Vital Records, 710 James Robertson Parkway, Nashville, TN 37243. Processing takes one to three weeks.

Online: Order through VitalChek, the only vendor Tennessee authorizes for online birth certificate orders. VitalChek adds $10.00 to the $15.00 state fee. An optional $5.00 expedite is available. See the state fee schedule for current pricing.

Tennessee Vital Records System

Tennessee vital records approved sources Oak Ridge Anderson County

The Tennessee Office of Vital Records operates the statewide system for all certified birth certificates. This centralized system means every county health department in Tennessee can access the same database. The Anderson County office in Clinton is a full-service location that can serve Oak Ridge residents for any Tennessee birth certificate request.

Tennessee's vital records system is regulated under Tenn. Code Ann. ยง 68-3-205, which restricts access to records less than 100 years old to eligible family members and legal representatives. Records 100 years or older are public. The full administrative regulations governing procedures and access are available through the Tennessee vital records regulations at Cornell Law School's legal information database.

Who Can Request a Birth Record

Not everyone qualifies for a certified copy of a birth record less than 100 years old. Tennessee law limits access to specific people.

Eligible requesters include the person named on the certificate (if 18 or older), a parent listed on the record, a legal guardian with court documentation, a spouse, a child, or a sibling of the named person. Legal representatives with written authorization from an eligible person may also apply. A valid government-issued photo ID is required. If requesting for another person, bring documentation showing the relationship: a birth certificate, marriage certificate, or court order as appropriate.

Records 100 years old or older are open to anyone. The state genealogy research guide explains how to access older records through TSLA and the TEVA database.

Historical Anderson County Birth Records

Tennessee birth certificates Anderson County Oak Ridge historical records

Statewide birth registration in Tennessee began in 1908. For Anderson County births before that year, the Tennessee State Library and Archives (TSLA) at 1001 Rep. John Lewis Way N., Nashville holds the relevant historical records. The collection includes early Anderson County vital records, church registers, and family documents. TSLA is open Tuesday through Saturday, 8:00 AM to 4:00 PM.

For births from 1908 through the early 1910s, the Tennessee Early Vital Records Application (TEVA) is a free online searchable database. Those early records are now over 100 years old and accessible to anyone. TEVA is searchable by name and county and is a good first step for genealogical research on early Anderson County families, including those in the Oak Ridge area before the city's formal founding in 1942.

Anderson County itself was established in 1801. Early records from the 19th century may be found at TSLA in land records, wills, and family papers. The CDC guide to Tennessee vital records is a useful starting point for anyone researching Tennessee records for the first time.
